Children and young people experience transitional periods as they grow and develop through the different stages in their live. This process unfolds over several years and involves significant emotional, physical, intellectual and physiological changes. To achieve their full potential young adults often require support in different areas of their lives. At GSC Through-Care we provide the following services which hold the young adult’s wellbeing at the centre.
River View Cottage has been designed to allow the young adults to develop skills required to allow them to live independently, with the support of staff if/when required.
- Staff will provide a safe environment, offering care and support to the young adults to develop their independent living skills through the implementation of the Life Skills Model.
- There will be a continuum of care to the young adults through the support from staff who have developed positive professional and effective relationships.
- Staff will encourage and support the young adults to have confidence in their problem solving, decision making and social skills, empowering them to develop their social networks and increase their social inclusion within the community.
- Our person centred approach ensures the young adults are fully involved in the decision making process required in the development of a bespoke individual throughcare/pathway plan.
- Young adults will be supported to access the following: Tenancy & Citizenship Award, Duke of Edinburgh Award, John Muir Award and Distance Learning qualifications.
- There will be opportunities to work in partnership with other agencies such as Skills Development Scotland and Community Health Services to enhance the young adult’s social/educational and vocational aspirations.
- Staff will support young adults through the transition into further education, training and employment.
- Each young adult will have their own outcomes framework. This will be measured every 10 weeks using various tools to measure outcomes; this includes a young adult’s subjective online assessment across the GIRFEC wellbeing indicators (including Hope).
- There will be support from a Transitions Coordinator to access a range of supports and resources for young adults that will assist them in adapting and settling into the routine of community life.
- Staff will work in close partnership with parent(s), carers, local authority services and the voluntary sector to ensure that a return to the community is a seamless and cohesive transition.
